Medical records are documents that explain details about a patient's history, clinical finding, diagnostic testing, pre and post operative care, patient progress and medication.
If you want to transfer records from multiple doctors. You will need to print and complete the release form for each doctor.
If you are a new patient, please complete the medical release form(s) prior to your appointment. Please be sure to bring the form(s) with you to your first appointment.
If you are a current patient, please bring the form(s) to your next appointment or drop the form(s) off at the office.
We will fax the medical release form to the doctors. However if you have access to a fax machine you can fax the form(s) to your doctors directly.
