  1. Bring a valid picture ID.
  2. Bring your insurance card(s)
  3. Print and fill out the New Patient Forms and bring them to your first appointment.   
  4. Print and fill out the Medical Record Release Form to transfer your medical records to our office, if applicable.
  5. Arrive 15 minutes prior to your scheduled appointment.
